The stotra appears in the Siddheshwari Tantra as part of a dialogue between Shiva (Maheshwara) and Parvati (Uma). Moved by compassion for beings caught in the sufferings of worldly existence, Shiva reveals to the Goddess a hymn whose very purpose (apad-uddhara-hetukam) is to rescue devotees from every form of calamity. Each verse is a direct cry to the Divine Mother as the one and only refuge, culminating in the refrain 'Trahi Durge' — 'Save me, O Durga.'

Devotees through the ages have recited this hymn at moments of acute crisis — shipwreck, fire, attack by enemies, false imprisonment and sudden illness — and traditional accounts hold that those who called upon the Mother with even a single verse were unexpectedly delivered, the danger dissolving as if the Goddess herself had thrown them the 'boat of rescue' (nistara-nauka) the hymn describes.

ಶ್ಲೋಕ 1

ನಮಸ್ತೇ ಶರಣ್ಯೇ ಶಿವೇ ಸಾನುಕಮ್ಪೇ ನಮಸ್ತೇ ಜಗದ್ವ್ಯಾಪಿಕೇ ವಿಶ್ವರೂಪೇ। ನಮಸ್ತೇ ಜಗದ್ವನ್ದ್ಯಪಾದಾರವಿನ್ದೇ ನಮಸ್ತೇ ಜಗತ್ತಾರಿಣಿ ತ್ರಾಹಿ ದುರ್ಗೇ॥೧॥

namaste sharanye shive sanukampe namaste jagad-vyapike vishva-rupe | namaste jagad-vandya-padaravinde namaste jagat-tarini trahi durge ||1||

ಅರ್ಥ:Salutations to you, O Refuge of all, O auspicious and compassionate one; salutations to you who pervade the universe and whose form is the cosmos. Salutations to you whose lotus-feet are adored by the whole world — O Saviour of the world, protect me, O Durga!

ನಮಸ್ತೇ ಜಗಚ್ಚಿನ್ತ್ಯಮಾನಸ್ವರೂಪೇ ನಮಸ್ತೇ ಮಹಾಯೋಗಿವಿಜ್ಞಾನರೂಪೇ। ನಮಸ್ತೇ ನಮಸ್ತೇ ಸದಾನನ್ದರೂಪೇ ನಮಸ್ತೇ ಜಗತ್ತಾರಿಣಿ ತ್ರಾಹಿ ದುರ್ಗೇ॥೨॥

namaste jagac-chintyamana-svarupe namaste maha-yogi-vijnana-rupe | namaste namaste sadananda-rupe namaste jagat-tarini trahi durge ||2||

ಅರ್ಥ:Salutations to you whose true nature is beyond the thought of the world; salutations to you who are the wisdom of the great yogis; salutations again and again to you who are ever-blissful — O Saviour of the world, protect me, O Durga!

ಶ್ಲೋಕ 3

ಅನಾಥಸ್ಯ ದೀನಸ್ಯ ತೃಷ್ಣಾತುರಸ್ಯ ಭಯಾರ್ತಸ್ಯ ಭೀತಸ್ಯ ಬದ್ಧಸ್ಯ ಜನ್ತೋಃ। ತ್ವಮೇಕಾ ಗತಿರ್ದೇವಿ ನಿಸ್ತಾರಕರ್ತ್ರೀ ನಮಸ್ತೇ ಜಗತ್ತಾರಿಣಿ ತ್ರಾಹಿ ದುರ್ಗೇ॥೩॥

anathasya dinasya trishnaturasya bhayartasya bhitasya baddhasya jantoh | tvam eka gatir devi nistara-kartri namaste jagat-tarini trahi durge ||3||

ಅರ್ಥ:For the helpless, the wretched, the craving-tormented, the terrified, the frightened and the bound creature, you alone, O Goddess, are the recourse, the one who delivers — O Saviour of the world, protect me, O Durga!

ಶ್ಲೋಕ 4

ಅರಣ್ಯೇ ರಣೇ ದಾರುಣೇ ಶತ್ರುಮಧ್ಯೇ ಜಲೇ ಸಙ್ಕಟೇ ರಾಜಗೇಹೇ ಪ್ರವಾತೇ। ತ್ವಮೇಕಾ ಗತಿರ್ದೇವಿ ನಿಸ್ತಾರಹೇತು- ರ್ನಮಸ್ತೇ ಜಗತ್ತಾರಿಣಿ ತ್ರಾಹಿ ದುರ್ಗೇ॥೪॥

aranye rane darune shatru-madhye jale sankate raja-gehe pravate | tvam eka gatir devi nistara-hetur namaste jagat-tarini trahi durge ||4||

ಅರ್ಥ:In the forest, in battle, amid dreadful enemies, in water, in danger, in the king's court, in a storm — you alone, O Goddess, are the means of rescue — O Saviour of the world, protect me, O Durga!

ಶ್ಲೋಕ 5

ಅಪಾರೇ ಮಹದುಸ್ತರೇಽತ್ಯನ್ತಘೋರೇ ವಿಪತ್ಸಾಗರೇ ಮಜ್ಜತಾಂ ದೇಹಭಾಜಾಮ್। ತ್ವಮೇಕಾ ಗತಿರ್ದೇವಿ ನಿಸ್ತಾರನೌಕಾ ನಮಸ್ತೇ ಜಗತ್ತಾರಿಣಿ ತ್ರಾಹಿ ದುರ್ಗೇ॥೫॥

apare mahad-ustare 'tyanta-ghore vipat-sagare majjatam deha-bhajam | tvam eka gatir devi nistara-nauka namaste jagat-tarini trahi durge ||5||

ಅರ್ಥ:For embodied beings drowning in the boundless, impassable, utterly terrible ocean of calamity, you alone, O Goddess, are the boat of deliverance — O Saviour of the world, protect me, O Durga!

ನಮಶ್ಚಣ್ಡಿಕೇ ಚಣ್ಡದೋರ್ದಣ್ಡಲೀಲಾ- ಸಮುತ್ಖಣ್ಡಿತಾಖಣ್ಡಲಾಶೇಷಶತ್ರೋಃ। ತ್ವಮೇಕಾ ಗತಿರ್ವಿಘ್ನಸನ್ದೋಹಹರ್ತ್ರೀ ನಮಸ್ತೇ ಜಗತ್ತಾರಿಣಿ ತ್ರಾಹಿ ದುರ್ಗೇ॥೬॥

namash chandike chanda-dor-danda-lila- samutkhandita-khandalashesha-shatroh | tvam eka gatir vighna-sandoha-hartri namaste jagat-tarini trahi durge ||6||

ಅರ್ಥ:Salutations, O Chandika, who by your fierce sport of the mighty raised arm have cut down every last unbroken enemy; you alone are the recourse that removes the host of obstacles — O Saviour of the world, protect me, O Durga!

ಶ್ಲೋಕ 7

ತ್ವಮೇಕಾ ಸದಾರಾಧಿತಾ ಸತ್ಯವಾದಿ- ನ್ಯನೇಕಾಖಿಲಾ ಕ್ರೋಧನಾ ಕ್ರೋಧನಿಷ್ಠಾ। ಇಡಾ ಪಿಙ್ಗಲಾ ತ್ವಂ ಸುಷುಮ್ನಾ ನಾಡೀ ನಮಸ್ತೇ ಜಗತ್ತಾರಿಣಿ ತ್ರಾಹಿ ದುರ್ಗೇ॥೭॥

tvam eka sadaradhita satya-vadiny aneka-akhila krodhana krodha-nishtha | ida pingala tvam sushumna cha nadi namaste jagat-tarini trahi durge ||7||

ಅರ್ಥ:You alone are ever-worshipped, the truthful one; you are Ida, Pingala and Sushumna, the channels of the inner being — O Saviour of the world, protect me, O Durga!

ಶ್ಲೋಕ 8

ನಮೋ ದೇವಿ ದುರ್ಗೇ ಶಿವೇ ಭೀಮನಾದೇ ಸರಸ್ವತ್ಯರುನ್ಧತ್ಯಮೋಘಸ್ವರೂಪೇ। ವಿಭೂತಿಃ ಸತಾಂ ಕಾಲರಾತ್ರಿಸ್ವರೂಪೇ ನಮಸ್ತೇ ಜಗತ್ತಾರಿಣಿ ತ್ರಾಹಿ ದುರ್ಗೇ॥೮॥

namo devi durge shive bhima-nade sarasvaty-arundhaty-amogha-svarupe | vibhutih satam kala-ratri-svarupe namaste jagat-tarini trahi durge ||8||

ಅರ್ಥ:Salutations, O Goddess Durga, auspicious one of terrifying roar; you are Saraswati, Arundhati and the unfailing one; you are the glory of the noble, the very form of Kalaratri — O Saviour of the world, protect me, O Durga!

ಶ್ಲೋಕ 9

ಶರಣಮಸಿ ಸುರಾಣಾಂ ಸಿದ್ಧವಿದ್ಯಾಧರಾಣಾಂ ಮುನಿದನುಜವರಾಣಾಂ ವ್ಯಾಧಿಭಿಃ ಪೀಡಿತಾನಾಮ್। ನೃಪತಿಗೃಹಗತಾನಾಂ ದಸ್ಯುಭಿಸ್ತ್ರಾಸಿತಾನಾಂ ತ್ವಮಸಿ ಶರಣಮೇಕಾ ದೇವಿ ದುರ್ಗೇ ಪ್ರಸೀದ॥೯॥

sharanam asi suranam siddha-vidyadharanam muni-danuja-varanam vyadhibhih piditanam | nripati-griha-gatanam dasyubhis trasitanam tvam asi sharanam eka devi durge prasida ||9||

ಅರ್ಥ:You are the sole refuge of the gods, the Siddhas and Vidyadharas, of sages, of the best of men afflicted by disease, of those imprisoned in the king's house and those terrorised by robbers — you alone are the refuge, O Goddess Durga, be gracious!

ಶ್ಲೋಕ 10

ಇದಂ ಸ್ತೋತ್ರಂ ಮಯಾ ಪ್ರೋಕ್ತಮಾಪದುದ್ಧಾರಹೇತುಕಮ್। ತ್ರಿಸನ್ಧ್ಯಮೇಕಸನ್ಧ್ಯಂ ವಾ ಪಠನಾದ್ಘೋರಸಙ್ಕಟಾತ್॥

idam stotram maya proktam apad-uddhara-hetukam | tri-sandhyam eka-sandhyam va pathanad ghora-sankatat ||

ಅರ್ಥ:This hymn for deliverance from calamity has been spoken by me. Whoever recites it at the three junctures of the day, or even once, in dire distress — there is no doubt that on earth, in heaven or in the nether world he is freed; whoever with devotion always recites all of it or even a single verse abandons every evil deed and attains the supreme state.

ಶ್ಲೋಕ 11

ಮುಚ್ಯತೇ ನಾತ್ರ ಸನ್ದೇಹೋ ಭುವಿ ಸ್ವರ್ಗೇ ರಸಾತಲೇ। ಸರ್ವಂ ವಾ ಶ್ಲೋಕಮೇಕಂ ವಾ ಯಃ ಪಠೇದ್ಭಕ್ತಿಮಾನ್ಸದಾ। ಸರ್ವಂ ದುಷ್ಕೃತಂ ತ್ಯಕ್ತ್ವಾ ಪ್ರಾಪ್ನೋತಿ ಪರಮಂ ಪದಮ್॥

The stotra itself prescribes recitation at the three junctures of the day (trisandhyam) or at least once daily. In an emergency it may be chanted then and there with full faith, even a single verse. Sit facing east before an image of Durga if possible, light a lamp, and recite with concentration on the refrain 'Trahi Durge' (protect me, O Durga), feeling oneself wholly surrendered to the Mother. Regular recitation builds an enduring shield of protection.

Apad-uddharaka means 'one who lifts up out of calamity'. Apad = misfortune/danger, uddharaka = rescuer. The stotra is recited to be delivered from sudden dangers, disasters and grave distress of every kind.
It is drawn from the Siddheshwari Tantra, framed as a dialogue (Uma-Maheshwara Samvada) in which Lord Shiva teaches the hymn to Goddess Parvati. It is therefore regarded as having come directly from Shiva himself.
Traditionally it is recited at the three sandhyas (dawn, noon and dusk), or at least once a day. It is especially recited at the moment of facing any danger — illness, travel, conflict, legal trouble or natural calamity.
Yes. The closing phalashruti states that whoever recites the whole hymn or even a single verse with devotion is freed from distress and evil deeds and attains the supreme state, so even one verse chanted in faith is held to be efficacious.
